I suspect these days there's a lot of diagnosis of the former, and trying to find the latter by medical interventions...

the term "cone of depression" always cracks me up.

Somehow, "cone of depression" gives me an image from the Abarat books, where people are walking around with these elaborate not-hats on their heads that are their dreams. I can imagine a cone of depression being a conical sort of felted hat of nothingness.
